Sub: Claim Repudiation of policy no. 301200/48/09/8500015602 , Claim no. HH 131107066


With reference to the above subject, We are the policy holder of National Insurance Co Ltd. Our policy number is 301200/48/09/8500015602

We had put claim file of mine for reimbursement on dtd. 1st Jan-2011. at Heritage Health TPA Pvt. Ltd. Ahmedabad. All the queries had been replied with supporting.

With reference to your letter , mine disease is not pre-existing disease .At the time of taking policy , I was free of any kind of diseases. As per the doctor certificate it also reflect that my blood pressure disease occurred after taking my mediclaim policy . Claim came in 22nd month of policy. So its not pre-existing disease . So policy condition 4.01 does not applicable to me.

So I should get my claim amount.

You are kindly requested to look into this and do the needful at the earliest .
