Sub: Claim Repudiate under the policy no. 301200/48/09/8500004277 ( Claim no. HH131105360) from National Insurance Co. Ltd.

With reference to the above subject, We are the policy holder of National Insurance Co Ltd. Our policy number is 301200/48/09/8500011729

We had put claim file of my wife “Ranjanben” for reimbursement on dtd. 28/09/2010. at Heritage Health TPA Pvt. Ltd. Ahmedabad. My claim has been treated as no claim under the mediclaim policy condition 4.3


the letter from the treating doctor & letter says that it was accidental case. & as per Mediclaim insurance (individual) condition 4.3, surgery for prolapsed intervertebral disc unless arising from accident . My wife surgery arising from the accidental injury. So I must get my claim.
