Dear Sir/Madam

Sub: Claim Repudiate under the policy no. 301200/48/09/8500004277 ( Claim no. HH131102899) from National Insurance Co. Ltd.

With reference to the above subject, We are the policy holder of National Insurance Co Ltd. Our policy number is 301200/48/09/8500004277

We had put claim file of my wife ”Madhuben” for reimbursement at Heritage Health TPA Pvt. Ltd. Ahmedabad. All the queries had been replied with supporting.

We have got the claim repudiation letter from National Insurance co. Ltd. On dtd.08/08/2011 ref no.301200/Mediclaim/HHI131102899 that Parenteral Chemotherapy is not payable under the policy condition no.2.6

But sir, as you know the in cancer treatment particularly in Chemotherapy hospitalization is not needed. So my claim shall be payable.

You are kindly requested to look into this and do the needful at the earliest .
