Dear Sir,

Sub: Claim Repudiation Mediclaim policy no.141100/48/2010/5576 from The Oriental Insurance Co. Ltd.

With reference to above subject, I undersigned Nalin patel had taken Mediclaim policy from The Oriental Insurance Company. My policy no. is 141100/48/2010/5576 . My policy was continued since last so many years.

We had put claim file for reimbursement on 25th March-2010,We have come to know that our claim has been repudiated due to “Late Submission of Claim Papers”, Intimation was given on time.
