Dear Sir

Sub: Lost of cheque of Mediclaim renewal with renewal notice from your DO-4 Office , Ahmedabdad of policy no 301200/48/09/8500013416 & 301200/48/09/8500013417& Continuous benefit of my mediclaim policy

With reference to the above subject. I undersigned Kishanbhai Hirabhai Patel had above mentioned mediclaim policies since last 5 years . My servicing branch is National Insurance Co. Ltd. DO-IV. Ahmedabdad.(301200) I had given renewal cheque on dtd.19th January-2011 of following details. Cheque no.235338, The Ahmedabdad Dist. Co. Op. Bank., Thaltej Branch of Rs.14015/-
In this regard, we have written a email to. Mr. S K. Biswas at email address . sk.biswas@nic.co.in on dtd. 14th March-2011. till date your office has not given any reply to us. Even we have complaints online through your website on dtd.16th April-2011. Grievance Number: GRI110100201. No reply from your side.
I am sure , you will be surprised the know that, there is no Inward Mechanism exist in your Divisional office no.4, No inward number is given, Inward register is not maintained, even policy renewal acknowledgement also not given. In this scenario ,my renewal cheque has been lost by your branch authorities.

As per the information provided by National insurance DO-4, office under the RTI act ref no. 301200/Mediclaim/HH/2011/rcs RFN 2171,dtd.20/07/2011 , it self they written that ”There is no such system for maintain new proposal or policy renewal register as per the official manual”

So, above self confessed letter from National Insurance Co. Ltd. DO-4, Ahmedabdad say that they are not maintaining inward mechanism for proposal & renewal notice.
In such scenario my mediclaim renewal cheque has been lost by the National Insurance Co. Ltd. DO-4, Ahmedabdad officials.

Its fault from your company, why we as a customer suffer? You must give us continuous benefit of our mediclaim policies. As you do not have policy renewal inward mechanism.

If you check my record of paying mediclaim premium date, it been paid well before in advance.

We have come to know that our all the policies have been lapsed . And our mediclaim polices have been treated as a fresh policies.

Looking at the fact , you are requested to consider our policies as continuous policies. As current year premium has been paid by cheque no.235339, ADC Bank. Dtd.09/03/2011.

You are requested to give us the justice in this regard.
