Sir, I beg to state that, Mr. Vikram Singh (Mobile No, 09716087852) has phone to me that, he is from I R D A and he talk with me on behalf of Birla Sun Life Insurance and accordingly he tell me the sum of Rs.900,000/= (nine Lakh) are Bonus senction against my Birla Sun Life Policy No. 005799008 & 003655773 and advice me for deposit some amount as Bonus Security & as per Mr. Vikram Singh I am deposited amount by 4(four) nos. United Bank of India Chq. No. 418117 dtd. 05.08.2013 of Rs.25,000/=, Chq. No. 418120 dtd. 28.08.2013 of Rs. 30,000/=, Chq No. 269961 dtd.28.08.2013 of Rs. 30,000/= & Chq. No. 269962 dtd.30.08.2013 of Rs. 25,000/=. But regret to inform you that,Mr. Vikram Singh send me 3(three) nos. Burla Sun Life Insurance Policy No. 006205347,against Chq. No. 418117 dtd. 05.08.2013, Policy No. 006245196 against Chq. No. 418120 dtd. 28.08.2013 & Policy No. 006247117 against Chq. No. 269961 dtd. 28.08.2013 inplace of senctioned Bonus which was confirm by Mr. Vikram Singh I have not received any document against my Chq No. 269962 dtd. 30.08.2013 from Mr. Vikram Singh.

In this connection I like to intimate you that, all the Policy Policy signature are FALSE, because I have not sign any form & documents I have only send My address proof & pass port size photos alongwith cancelled Cheque No. 418118 & 418119 and amount Cheques No. are mention as above to Mr. Vikram Singh.

Furthe note that, Mr. Vikram Singh again phone to me and tell me for send another some amount for security deposit against senctioned Bonus in favour of Mr. Ravi Shankar A/c. no. 20761050006401 IFSC code: HDFC0002076, Mr. Birender Kumar Delhi, & Mr. Mukesh Kumar A/c. No.910010021851448 & accoudingly I am deposit the amount of Rs. 25,000/= Chq. No. 269970 dtd. 05.12.2013 & amount of Rs 23,700/= by Chq No. 269971 dtd. 26.12.2013 to Mr. Ravi Shanakar's account & the amount of Rs. 42.200/= by Chq No. 269972 dtd. 16.01.2014 to Mr. Birender Kumar's account & the amount of Rs. 25,000/= by Chq No. 269976 dtd. 05.03.2014 to Mr. Mukesh Kumar's account all Cheque s are diposited against Bonus Security Deposit But Regret to inform you that, till date I have not receive any Bonus amount or any Bonus senction letter from the Birla Sun Life Insurance Co. Ltd. against my above Cheques.

Sir I hopefullt request you to kindly look into my above matter and arrange to refund of my diposited amount which was diposited by me through Bank Cheques in several date & time & also note that, all are Insurance Broker or Advisor are misguide & Cheat with the Insurance Client & Customer.

Hope you will look in to the above matter & also take necessary step or Legal action against Insurance Broker or Advisor. I awaiting for your early reply.
